On behalf of children aged under 13, Parents/Guardians should confirm their agreement with and willingness to adhere to the statements listed below by completing and submitting this form. Before completing, please read the school policy for the use of photographs, video, internet and ipad.

I confirm that I have read and understand the school policy for the use of Photographs & Video in relation to my child and the use of Internet and ipads within school by my child and that I am submitting this form on behalf of my child

. I will access the system with the login and password, which I keep secret;

• I will not access other people’s files;

• I will only use the computers / tablets for school work and homework;

• I will not bring in memory sticks from outside school unless I have been given permission;

• I will ask permission from a member of staff before using the Internet;

• I will only e-mail people I know, or my teacher has approved;

• The messages I send will be polite and responsible;

• I will not give my home address or telephone number, or arrange to meet someone, unless my parent, carer or teacher has given permission;

• I will report any unpleasant material or messages sent to me. I understand my report would be confidential and would help protect other pupils and myself;

• I understand that the school check my computer files and may monitor Internet sites I visit.

• I will take good care of the iPads

• I will never leave any iPad unattended

• I will keep food and drinks away from my iPad since they may cause damage to the device

• I will not disassemble any part of any iPad or attempt any repairs

• I will protect the iPad by only carrying it whilst it is in a case

• I will use the iPad in ways that are appropriate

• I understand that the iPad is subject to inspection at any time without notice

• I will only photograph people with their permission

• I will only use the camera or the microphone when my teacher allows me to

• I will never share any images or movies of people in a public space on the Internet, unless I am asked to do so by my teacher

• I agree to abide by the statements of this iPad acceptable usage policy
